During abdominoplasty, the plastic surgeon makes an incision that extends from hip to hip along the lower pubic area. After breast reduction, you’ll be wrapped in an elastic bandage or a surgical bra over gauze dressings. A small tube may be placed in each breast to drain off blood and fluids for the first day or two. You may feel some pain for the first couple of days-especially when you move around or cough-and some discomfort for a week or more.
The bandages will be removed a day or two after surgery, though you’ll continue wearing the surgical bra around the clock for several weeks, until the swelling and bruising subside. Your stitches will be removed in one to three weeks. You may be up and about in a day or two, but your breasts may still ache occasionally for a couple of weeks. You should avoid lifting or pushing anything heavy for three or four weeks.
Although much of the swelling and bruising will disappear in the first few weeks, it may be six months to a year before your breasts settle into their new shape. Even then, their shape may fluctuate in response to your hormonal shifts, weight changes, and pregnancy.

A tummy tuck is performed by first carefully marking the patient while they are standing. Careful pre-operative marking is essential to guiding the surgery to success. The procedure begins just above the pubis, by lifting the skin off the muscle from the pubic bone up to the bottom of the ribs. This releases the skin to allow the lower abdominal excess to be removed and the upper abdominal skin to be stretched downward to fill in the space from where the skin is removed. It also allows the surgeon to easily visualize the stretched out, thinned or separated muscles and to tighten them. Although the procedure does require an incision, many plastic surgeons’ experience and intimate understanding of the patient’s anatomy and body contours will allow the scar to be well hidden in a bikini bathing suit or even skimpy underwear. Like all procedures, Abdominoplasty does carry risks, including the risk of infection, bleeding and the risk from anesthetics. It is important to note that under the care of a Board Certified Plastic Surgeon, great care will be taken to avoid or minimize these risks.

Many plastic surgeons use a “tumescent technique.” After the induction of general anesthesia or intravenous sedation, a large volume, up to several quarts, of very dilute local anesthetic is injected into the areas to be treated. There is a tiny dose of adrenaline in the fluid as well, which makes the blood vessels in the area clamp down. This prevents perhaps 95% of the blood loss that would otherwise occur during liposuction.
Thin hollow stainless steel tubes are placed through short (perhaps 1/4″) incisions in the area treated and are used to suction away the appropriate amount of fat. Sheets of lightweight foam are placed on the treated areas, and a compression garment, like a corset, is placed. Bruising is usually scant.

If You’re Considering Scar Revision…
Scars, whether they’re caused by accidents or by surgery, are unpredictable. The way a scar develops depends as much on how your body heals as it does on the original injury or on the surgeon’s skills.
Many variables can affect the severity of scarring, including the size and depth of the wound, the blood supply to the area, the thickness and color of your skin, and the direction of the scar. How much the appearance of a scar bothers you is, of course, a personal matter.
While no scar can be removed completely, plastic surgeons can often improve the appearance of a scar, making it less obvious through the injection or application of certain steroid medications or through surgical procedures known as scar revisions.
If you’re considering scar revision, this will give you a basic understanding of the most common types of scars, the procedures used to treat them, and the results you can expect. It can’t answer all of your questions, since a lot depends on your individual circumstances. Please be sure to ask your doctor if there is anything about the procedure you don’t understand.
After Scar Revision
With any kind or scar revision, it’s very important to follow your surgeon’s instructions after surgery to make sure the wound heals properly. Although you may be up and about very quickly, your surgeon will advise you on gradually resuming your normal activities.
As you heal, keep in mind that no scar can be removed completely; the degree of improvement depends on the size and direction of your scar, the nature and quality of your skin, and how well you care for the wound after the operation. If your scar looks worse at first, don’t panic-the final results of your surgery may not be apparent for a year or more.

“Doc, I hate these wings!” This is a common refrain heard by many Los Angeles Plastic Surgeons, from patients who complain about the redundant, excess skin and fat of their upper arms. There are many reasons why patients develop this problem. Weight loss, the aging process, after Gastric Bypass Surgery and various medical conditions all can be the cause. The common complaint is that the skin and soft tissues hang creating an unsightly appearance even when the arms are fully covered. Short sleeve shirts and blouses can leave patients feeling embarrassed and constantly aware of the issue, but there is hope! An Arm Lift, or Liposuction, or both, may be the answer.

An Arm Lift is usually performed under general anesthesia. Before surgery, the amount of skin and fat to be removed will be estimated and markings will be placed accordingly. Unlike most physicians, Dr. Lent places the incision on the underside of the arm in a location making the resulting scars as inconspicuous as possible. On some occasions the incision can be placed in the Axilla (armpit). Prior to surgery, all patients are asked to avoid any medication, which thins the blood, such as Aspirin, for at least two weeks. Patients are told to avoid exercise and heavy lifting for at least six weeks after surgery.

Like all medical treatment an Arm Lift is generally a safe procedure, but there are risks. The risks include bleeding, infection, skin injury and the risk of anesthesia. Under the care of a Board Certified Los Angeles Plastic Surgeon, like Dr. Lent, every effort will be made to prevent and minimize risk to make your outcome a successful one. It is very important that smokers quit smoking and using nicotine products for at least one month before and for at least one month after surgery.

Breast augmentation (or augmentation mammaplasty) is a cosmetic procedure that uses implants to enlarge and shape the breasts. Some of the reasons women seek breast enlargement surgery are to change the size or shape of their breasts, to bring balance when a breast that is smaller than the other, and to correct a loss of volume or fullness after weight loss or pregnancy.

Some women benefit from having a mastopexy (breast lift) along with the augmentation when the breasts are sagging.

The Procedure
Breast augmentation surgery is performed on an outpatient basis under general anesthesia and usually takes less than 2 hours. The incisions are small and usually located underneath the breast just above the crease or around the lower edge of the areola to keep scars as inconspicuous as possible. After the incision has been made, the surgeon creates a pocket into which the implant is inserted; either directly behind the breast tissue or beneath the muscle.

About the Implants
Most women receive saline-filled implants which are filled with sterile saltwater. The implants may also be filled with silicone gel, although the use of these implants is limited to patients who fill certain requirements. Recovery
Upon leaving the operating room you will be wearing a supportive, elastic bra. Initial discomfort will be controlled with oral medication. Within a day or two you should be up and around and can resume light activities. After several weeks most of the swelling will be gone, exercise can be resumed and you will start to see your final results.

Results
Most women who have breast enlargement surgery are very pleased with the results and find that their new figure is more proportioned and gives them a greater sense of self-confidence.

Risks
The risks associated with breast enlargement surgery are not great. However, it should be noted that a small percentage of women may experience capsular contracture (a hardening of the scar tissue that forms around the implant which may require a surgical revision.) It is also important to keep in mind that most implants have to be replaced at some point within a woman’s lifetime as they are not life-long devices.

The new CO2 pulse lasers are effective for treating fine wrinkles that occur with aging and exposure to the elements. We are using them for blepharoplasty and facial resurfacing either with or without facelift surgery. The laser also treats the vertical wrinkles that occur in the upper lip area. This new method for Laser Skin Resurfacing takes advantage of the abilitt of the laser to penetrate the skin to a precise depth. The consistency and degree of penetration affords an even effect over the entire treated area. The superficial skin is shed and new skin regenerates, providing a fresher, smoother appearance.

After the treatment the skin resembles a deep sunburn for about ten days. The redness fades and turns pink within a few weeks. Makeup can easily camouflage the pink color which disappears in one to three months.

During the healing process, we advise that patients avoid prolonged exposure to the sun.

Potential postoperative complications of pigmentary changes of the skin are managed through the preoperative and postoperative use of hydraquinone and retinoid creams. Introduction:
Liposuction is a cosmetic surgery operation that helps sculpt the body by removing fat from specific areas like the abdomen, hips, buttocks, thighs, knees, upper arms, chin, cheeks, and neck. The fat is usually removed through a cannula (a hollow tube) and aspirator (a suction device). Liposuction is not a substitute for dieting and exercise, but it can remove stubborn areas of fat that do not respond to traditional weight loss methods.

Best candidates for liposuction:

Eyelid surgery (technically called blepharoplasty) is a procedure to remove fat–usually along with excess skin and muscle from the upper and lower eyelids. Eyelid surgery can correct drooping upper lids and puffy bags below your eyes - features that make you look older and more tired than you feel, and may even interfere with your vision. However, it won’t remove crow’s feet or other wrinkles, eliminate dark circles under your eyes, or lift sagging eyebrows. Rhinoplasty is nasal surgery which is designed to improve the appearance of the nose, and/or the function of the nose in regards to breathing.

Rhinoplasty is often considered the most challenging type of plastic surgery, and choosing the exact procedure involves studying the relationship of the nose to the rest of the face, and reshaping the nose to enhance facial balance. No other plastic surgery is individualized to the degree that rhinoplasty must be. A thorough examination, careful discussion of the patient’s goals, and a review of photographic imaging are important in establishing the surgical plan. I usually recommend at least two separate preoperative consultations, to ensure clear communication between patient and surgeon, and to allow an accurate and complete analysis of the patient’s goals and nasal anatomy.

Either ‘closed’ or ‘open’ techniques of rhinoplasty may be performed. The closed technique uses only incisions inside the nostrils, and may suffice for more straight-forward rhinoplasties, for instance a first-time or ‘primary’ rhinoplasty that involves the reduction of a dorsal nasal prominence or ‘hump.’ In most cases, rhinoplasties will involve precise modification of the nasal tip, which is best performed using the ‘open’ technique. This procedure uses the same incisions as the closed technique, and adds a small incision across the columella (the column of tissue between the nostrils). A small external scar results, but this typically heals very well and can be nearly imperceptible. The advantage of the open technique is that is allows the surgeon to better visualize the framework of bone and cartilage that gives the nose its shape. The framework can be precisely altered with the open technique, so as to achieve the desired result with enhanced reliability and predictability.

Depending on the needs of the patient, rhinoplasty may involve the removal or repositioning of cartilage and bone, or the addition of cartilage from the nasal septum or ear. Other tissues, or even synthetic materials, are sometimes used.

When nasal surgery is performed to enhance breathing, procedures may include: septoplasty (modification of the nasal septum, which is the plate of cartilage between the right and left nasal passages), turbinate surgery (modification of tissues deep within the nasal cavity) , or alteration of the nasal framework– most often in an area of the upper nasal passages termed the ‘internal valve.’

Rhinoplasty is performed as an outpatient procedure, and under a general anesthetic in most cases. After the surgery, a splint is usually placed over the nose for protection and stabilization, and packing may be used in the nostrils if the nasal passages or nasal bones are altered. Pain and discomfort following the surgery are controlled with oral pain medication, and the stitches and splint are removed within one week after surgery. Nasal packing, when used, is removed within one to three days. Most patients feel comfortable returning to work within one to two weeks. Most bruising resolves within two weeks, though more subtle swelling may persist for longer periods.

Women with very large, pendulous breasts may experience a variety of medical problems caused by the excessive weight-from back and neck pain and skin irritation to skeletal deformities and breathing problems.

Breast reduction, technically known as reduction mammaplasty, is designed for such women. The procedure removes fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer. It can also reduce the size of the areola, the darker skin surrounding the nipple. The goal is to give the woman smaller, better-shaped breasts in proportion with the rest of her body.

The Best Candidates for Breast Augmentation
Breast reduction is usually performed for physical relief rather than simply cosmetic improvement. Most women who have the surgery are troubled by very large, sagging breasts that restrict their activities and cause them physical discomfort.

In most cases, breast reduction isn’t performed until a woman’s breasts are fully developed; however, it can be done earlier if large breasts are causing serious physical discomfort. The best candidates are those who are mature enough to fully understand the procedure and have realistic expectations about the results. Breast reduction is not recommended for women who intend to breast-feed.

All Surgery Carries Some Uncertainty and Risk
Breast reduction is not a simple operation, but it’s normally safe when performed by a qualified plastic surgeon. Nevertheless, as with any surgery, there is always a possibility of complications, including bleeding, infection, or reaction to the anesthesia. You can reduce your risks by closely following your physician’s advice both before and after surgery.

The procedure does leave noticeable, permanent scars, although they’ll be covered by your bra or bathing suit. (Poor healing and wider scars are more common in smokers.) The procedure can also leave you with slightly mismatched breasts or unevenly positioned nipples. Future breast-feeding may not be possible, since the surgery removes many of the milk ducts leading to the nipples.

Planning Your Surgery
In your initial consultation, it’s important to discuss your expectations frankly with your surgeon, and to listen to his opinion. Every patient-and every physician, as well-has a different view of what is a desirable size and shape for breasts.

Your surgeon will describe the procedure in detail, explaining its risks and limitations and making sure you understand the scarring that will result. The surgeon will also explain the anesthesia he or she will use, the facility where the surgery will be performed, and the costs.

He will discuss the variables that may affect the procedure-such as your age, the size and shape of your breasts, and the condition of your skin. You should also discuss where the nipple and areola will be positioned; they’ll be moved higher during the procedure, and should be approximately even with the crease beneath your breasts.
