Occasionally, an eyelid lift may not go as planned. According to Dr. Francis Palmer, a Beverly Hills plastic surgeon, eyelid surgery can also be done as a revision or secondary procedure to remove skin and fatty tissue. He discusses that this procedure may be performed as a result of fat and skin that were missed in the initial blepharoplasty procedure. Also, it may be performed after a few years where aging has occurred and the tissues have again fallen. He also explains that residual fat is common in the inside portion of the upper eyelid and outer portion of the lower eyelid.

I was looking for information on cosmetic eyelid surgery, specifically how long the surgery takes to perform. I ended up looking online and found a Utah plastic surgeon’s website. It says, “The surgery generally takes about one to three hours and is most often performed in the physician’s office. If additional cosmetic procedures are being performed, the eyelid surgery will most likely last longer.” That was actually longer than I had expected, but I guess for the best results it takes time.

I found a Kansas plastic surgeon with an answer. He says, “Your eyelid surgery may be performed in a hospital, free-standing ambulatory facility or office-based surgical suite.”

I’m receiving a blepharoplasty in the fall and I have been wondering where the incision is placed and if the scar is visible. I looked around on the internet for an answer and found a West Palm Beach plastic surgeon’s website. I’m very pleased that it says, “The upper eyelid incision is hidden very well back within the deep crease that is seen when the eye is shut, but retracts back when the eyelid is open.”

Typically the incisions follow the natural lines of your eyelids; in the creases of your upper lids, and just below the lashes in the lower lids. The eyelid surgery incisions may extend into the crow’s feet or laugh lines at the outer corners of your eyes. Working through these incisions, the surgeon separates the skin from underlying fatty tissue and muscle, removes excess fat, and often trims sagging skin and muscle. The incisions are then closed with very fine sutures.

Oftentimes patients complain that they appear more tired than they feel. Sometimes fat is repositioned from the lower eyelid into the deep tear troughs or the deep depressions that exist at the bottom of the lower eyelids. The incisions are easily hidden below the eyelashes of the lower eyelids and sometimes performed with no incision on the lower eyelid whatsoever, called a transconjunctival blepharoplasty. The upper eyelid incision is hidden very well back within the deep crease that is seen when the eye is shut, but retracts back when the eyelid is open. Many times eyelid surgery is performed at the same time as a face lift or a brow lift.

Traditional Blepharoplasty: An incision is made in the crease of the upper lid toward the temples. Once healed, it is not noticeable when the eye is open. On the lower lid, the incision is made along the lash line toward the temples, and is hidden by lashes when healed.
The Transconjunctival Approach: This is a newer technique for blepharoplasty, involving an incision inside the lower lid. Fat can be removed through this incision, but excess skin cannot be eliminated.

The blepharoplasty operation is designed to improve your appearance as much as possible. One must accept the judgment of your surgeon and realize that he or she will remove all the excess tissue to effect the optimum result for each patient. If a patient has very droopy brows, it may be impossible to obtain a significant improvement of upper eyelid appearance without first raising the brows. The blepharoplasty (without a browlift) does not affect droopy eyebrows. Your surgeon may suggest a browlift with you if he or she feels that your most significant problem is brow drooping.

Furthermore, psychological studies show that 50 percent of our initial impression of a person is made on the basis of the appearance of the eyes. The eyes tell others if we are sad, happy, angry or tired. Blepharoplasty corrects the appearance of droopy upper eyelids and puffy bags of the lower eyelids, and it produces dramatic and predictable results. It is very common to hear family and friends of eyelid patients compliment them that they look more rested and relaxed. Quite often, even these close acquaintances do not realize that surgery was performed. The recovery is very rapid and you can resume a normal routine in one to two days. The exotic look of a surgically revised asian eye has been perfected by some plastic surgeons. The procedure involves creating a pleasing double fold above the single eyelid. This procedure widens the eye into a beautifully shaped almond. Fine sutures leave no significant evidence of surgery.
I had no idea they had a completely different surgery for Asian eyelids. I wonder if it is more difficult than American eyelids. Has anyone had an experience with this that they would like to share?

The chemical peel is a widely accepted procedure to eradicate fine wrinkles in the skin. The procedure can be performed alone or at a later date after face lift or eyelid surgery.
Patients with fair complexions are better candidates as the chemical peel can remove pigment from the skin creating a line of demarcation between treated and untreated skin in patients with darker complexions.
The chemical peel creates a raw surface which heals in approximately ten days. Once healed, makeup can be applied. The peeled skin is initially red and rapidly fades to pink. This pink color may last for several months. Care must be taken to protect this skin from sun exposure by using a sun screen for at least six months.
Light chemical peels can be performed in the office. Deeper or full face peels are performed on an outpatient basis in a surgical center.

A forehead lift addresses the area of the upper face. It restores a more youthful, refreshed look to the area, making you look less tired, angry, or sad, by correcting drooping brows and improving the horizontal lines on your forehead. It is frequently performed with an upper lid blepharoplasty and can be performed with a face lift to provide facial balance and harmony.
The goal of the procedure is to raise the brow to a more aesthetically appealing position. It will also improve fullness over the outside corner of the lid, a scowling appearance of the eyes and horizontal frown lines of the forehead. It is not intended to create a look of surprise or what is commonly called the “deer in the headlights” look. Botox is not a substitute for a forehead lift, it can give you an idea of what to expect. The new CO2 pulse lasers are effective for treating fine wrinkles that occur with aging and exposure to the elements. We are using them for blepharoplasty and facial resurfacing either with or without facelift surgery. The laser also treats the vertical wrinkles that occur in the upper lip area. This new method for Laser Skin Resurfacing takes advantage of the abilitt of the laser to penetrate the skin to a precise depth. The consistency and degree of penetration affords an even effect over the entire treated area. The superficial skin is shed and new skin regenerates, providing a fresher, smoother appearance.
After the treatment the skin resembles a deep sunburn for about ten days. The redness fades and turns pink within a few weeks. Makeup can easily camouflage the pink color which disappears in one to three months.
During the healing process, we advise that patients avoid prolonged exposure to the sun.
Potential postoperative complications of pigmentary changes of the skin are managed through the preoperative and postoperative use of hydraquinone and retinoid creams. Eyelid surgery (technically called blepharoplasty) is a procedure to remove fat–usually along with excess skin and muscle from the upper and lower eyelids. Eyelid surgery can correct drooping upper lids and puffy bags below your eyes - features that make you look older and more tired than you feel, and may even interfere with your vision. However, it won’t remove crow’s feet or other wrinkles, eliminate dark circles under your eyes, or lift sagging eyebrows. While it can add an upper eyelid crease to Asian eyes, it will not erase evidence of your ethnic or racial heritage. Blepharoplasty can be done alone, or in conjunction with other facial surgery procedures such as a facelift or browlift.
